BackgroundExercise is primarily sustained by energy derived from lipids, and glucose. Substrate utilization is the pattern by which these fuel sources are used during activity. There are many factors that influence substrate utilization. We aim to delineate the effect of exercise intensity and body composition on substrate utilization.ObjectiveThe objective of our study was to discern the differences in substrate utilization profiles during a maximal and submaximal graded exercise test, and to determine the extent to which body composition influences substrate utilization (...) during the exercise tests.MethodsA total of 27 male athletes, 32.5 ± 11 years of age, were recruited for this study. Body composition was analyzed using a bioelectrical impedance analyzer. Maximal and submaximal exercise tests were performed on a treadmill. A novel graded submaximal treadmill protocol was used for the submaximal test.ResultsAverage percent body fat was 15.8 ± 5%. Average maximal oxygen consumption was 47.6 ± 9 mL/kg/min, while the average exercise intensity at which participants were shifting to glucose predominance for energy during the maximal and submaximal tests were 76 ± 8.3% and 58.4 ± 21.1%, respectively. A paired-samples t-test was conducted to compare percent VO2max at crossover point in maximal and submaximal graded exercise tests. There was a significant difference in percent VO2max at the crossover point for maximal and submaximal tests. A linear regression was performed to elucidate the interaction between exercise intensity at the crossover point and body composition during a maximal and submaximal graded exercise test. There was a significant effect of PBF on percent VO2max at crossover point during the maximal graded exercise test [F = 9.10, P = 0.006] with an R2 of 0.245. However, there was no significant effect of PBF on percent VO2max at crossover point during the submaximal graded exercise test.ConclusionSubstrate utilization, represented by the crossover point, is dependent on the rate of increase in exercise intensity. At maximal efforts, the crossover to carbohydrates from fats as the predominant fuel source occurs at a significantly later stage of percent VO2max than at submaximal efforts. Furthermore, body composition represented by PBF is a significant predictor of substrate utilization during maximal efforts. Athletes with a relatively higher PBF are more likely to have increased lipid oxidation during high intensity exercises than those with a lower body fat percentage. (shrink)

Les recherches montrent les impacts de la dépression maternelle sur le développement de l’enfant et la nécessité d’accompagner le processus de parentalité dans ce cas de figure. Après quelques repères théoriques autour de la dépression maternelle et/ou conjointe, l’article propose une thérapeutique qui s’est révélée efficace. Il s’agit d’Accordanse : un dispositif de soin et (...) de recherche accueillant des mères et leurs bébés en souffrance psychique centré sur la danse. Quelques séquences cliniques permettront de mieux mesurer dans quelle mesure la danse soutient la construction d’une chorégraphie des interactions harmonieusement accordée lorsque celle-ci est « empêchée » par la lourdeur de la dépression. The primary objective was to review pediatric ethics consultations at a large academic health center over a nine year period, assessing demographics, ethical issues, and consultant intervention. The secondary objective was to describe the evolution of PECs at our institution. This was a retrospective review of Consultation Summary Sheets compiled for PECs at our Academic Health Center between January 2008 and April 2017. There were 165 PECs reviewed during the study period. Most consult requests came from the inpatient setting, with (...) the Pediatric and Neonatal Intensive Care Units being the highest utilizers. Consultation utilization increased over the study period. The most common patient age was less than one year. Physicians were most likely to request consultation. Patient Best Interest, Withholding/Withdrawing of Life Sustaining Therapy, and Provider Moral Distress were ethical issues most commonly identified by the consultants. Making recommendations was the most common consultant intervention. The ethics consultation process evolved over time from informal provider discussions, to a hospital infant care review committee, to a pediatric only consultation service, to a combined adult/pediatric consultation service, with variable levels of salary support for consultants. Ethics consultation requests are growing at our institution. Similarities in identified ethical issues exist between our findings and existing literature, however meaningful comparisons remains elusive secondary to variability in approaches to investigation and reporting. A combined paid/volunteer/trainee ethics consultation service model appears sustainable and real time ethics consultation is feasible using this approach. (shrink)

In a clinic-wide approach to establish liberal policies, a closed psychiatric ward was planned to be opened. The leaders of the multi-professional team of this ward requested continuous ethics support during the first few months after the transition from their previously closed ward into an open one. During the process of accompanying the team through this ethically sensitive period of institutional change, several variations of ethics consultation were developed: the ‘context-adjusted’ clinical ethics support. Some ethics consultations focused on a retrospective (...) evaluation of a patient case, in other ethics consultations consolidation of a previous case discussion was worked out, and/or reflections on fundamental ethical issues were included. Based on our experiences and the feedback of the team, we consider this context-adjusted clinical ethics support as feasible and effective. (shrink)

Die forensische Kinder- und Jugendpsychiatrie steht in einem komplexen Spannungsfeld medizinischer, rechtlicher und sozialer Anforderungen. Die ethischen Herausforderungen, die sich daraus für den stationären Maßnahmenvollzug ergeben, sind bisher kaum untersucht, spezifische Hilfestellungen für Behandelnde fehlen. Diese Studie hat zum Ziel, ethische Themenfelder und Probleme in diesem Bereich zu identifizieren und ein Instrument zur Früherkennung und -intervention ethischer Probleme im Klinikalltag zu entwickeln. Methode: Eine systematische Literaturrecherche sowie eine Beobachtungstudie in der Jugendforensik der Universitären Psychiatrischen Kliniken Basel werden durchgeführt. Die Beobachtungsdaten (...) werden mittels qualitativer Inhaltsanalyse ausgewertet. Das Instrument wird auf der Basis der Resultate nach einer neuen Methode entwickelt. In der Literaturrecherche wurden 14 valide Publikationen identifiziert, die hauptsächlich ethische Probleme der Gerichtspsychiatrie thematisieren wie professionelle Rollenkonflikte, ethische Probleme der Diagnostik und Begutachtung oder der Umgang mit vertraulichen Informationen. Empirische oder empirisch-normative Studien fehlen. In der Beobachtungsstudie wurden 24 ethische Themen und spezifische Probleme identifiziert, besonders häufig sind ethische Fragen zu Moralkompetenzen der Jugendlichen, zur Behandlungsqualität, zum Umgang mit Regeln und Sanktionen und zur Freiheits- und Privatsphäre. Das Instrument zur Früherkennung und -intervention ethischer Probleme enthält die vier Kernelemente Risikofaktoren, Indikatoren, Interventionsplanung und Entscheidungsfindung. Der Maßnahmenvollzug der forensischen Kinder- und Jugendpsychiatrie offenbart ein breites Spektrum ethischer Themen mit spezifischen ethischen Problemen, welche in der Literatur bisher nur unzureichend untersucht sind. Das hier entwickelte Instrument FIEP unterstützt Behandelnde dabei, ethische Probleme frühzeitig zu erkennen und effizient zu bearbeiten. (shrink)

La presente investigación tuvo como objetivo realizar un análisis del procedimiento abreviado contemplado en el Código Orgánico Integral Penal (COIP), partiendo del criterio de que ciertos aspectos de este procedimiento vulneran derechos fundamentales del investigado. Se destaca particularmente el requisito para el procesado de admitir los hechos que se le imputan para acceder a este procedimiento, lo cual, según la opinión mayoritaria de doctrinarios, compromete el principio de no autoincriminación, la presunción de inocencia y el derecho a un juicio con (...) todas sus fases. El objetivo de esta investigación fue analizar el procedimiento abreviado establecido en el Código Orgánico Integral Penal (COIP), con el propósito de identificar posibles vulneraciones de derechos fundamentales del investigado. Se buscó específicamente evaluar cómo el requisito de admitir los hechos imputados para acceder a este procedimiento podría afectar principios como el de no autoincriminación y la presunción de inocencia. A través de un enfoque cualitativo y utilizando diferentes métodos de análisis, se procuró determinar si el procedimiento abreviado, tal como está configurado, garantiza un debido proceso conforme a los estándares de derechos humanos y constitucionales. La metodología aplicada se sustentó en un enfoque cualitativo, utilizando los métodos inductivo, analítico, sintético y exegético. Se llevó a cabo un análisis documental de la bibliografía relacionada con el tema investigado, así como de instrumentos internacionales de Derechos Humanos, la Constitución de la República de Ecuador, el COIP y jurisprudencia vinculada a la materia. Los resultados de la investigación determinaron que el procedimiento abreviado tal como se encuentra regulado en el COIP beneficia al Estado y al sistema de justicia al agilizar ciertos procesos legales. Sin embargo, se evidenció que este procedimiento puede vulnerar derechos esenciales del procesado, como el principio de no autoincriminación y la presunción de inocencia. Por lo tanto, se concluye que el procedimiento abreviado afecta el debido proceso al comprometer garantías fundamentales. La investigación señala que el procedimiento abreviado regulado en el COIP, si bien puede ser beneficioso para la eficiencia del sistema de justicia, también conlleva riesgos significativos de vulnerar derechos fundamentales del procesado. Por lo tanto, se destaca la necesidad de revisar y modificar este procedimiento para garantizar el respeto absoluto al debido proceso y a las garantías individuales del investigado. (shrink)

This article aims to show that the practice of attention can create an openness to the truth, from where ethics arises. It does so by exploring the role attention plays, according to Ricoeur, in Freud’s thought. Ricoeur shows how in the first stage of Freud’s thinking – that of the Project of a Scientific Psychology – attention is one of the instances in which a purely mechanical quantitative explanation can be questioned. Further on, with the introduction of narcissism, Ricœur shows (...) that attention, insofar as it opens a space for the “wounding truth,” opposes narcissism. Finally, the article explains how in the therapeutic setting an attentional epochē allows the therapist to be “the reality principle in flesh and in act,” so that the ego can gain control. According to Ricœur, this non-judgmental gaze opens a space of truthfulness for the patient’s self-knowledge which, although not being the totality of ethics, constitutes its threshold. (shrink)

This article updates a fundamental theme of the Conference of Puebla, the preferential option for the poor, through dialogue with the theme of aversion by the poor, of the philosopher Adela Cortina. Presents the meanings of the preferential option for the poor inaugurated in Medellín and solemnly proclaimed in Puebla and shows how this option was assumed by the posterior documents, both in Latin America and in the universal magisterium; then systematizes some aspects of the Aporophobia and, at the end, (...) proposes five theological-pastoral challenges: rediscover the option for the poor as Grace, realize the need for emotional involvement with the poor, discover the wealth of the poor and refuse the hate speech, reflect on ways to eliminate poverty and contribute to a new economy. In all this, the Magisterium of Francis is deeply illuminator. We can say that, with Pope Francis, a prophetic path of inclusion, coexistence and global responsibility is deepened, which turns visible and includes the poor and thus faces aporophobia. (shrink)

Creatures living under the rule of domestication form a communicative union based on shared morphological, behavioural, cognitive, and immunologicalresemblances. Domestic animals live under particular conditions that substantially differ from the original (natural) settings of their wild relatives. Here we focus on the fact that many parallel characters have appeared in various domestic forms that had been selected for different purposes. These characters are often unique for domestic animals and do not exist in wild forms. We argue that parallel similarities appear (...) in different groups in response to their interaction with theumwelt of a particular host. In zoosemiotic sense, the process of domestication represents a kind of interaction in which both sides are affected and eventuallytransformed in such a way that one is more integrated with the other than in the time of initial encounter. To support the claim that the approximate number system represents rational numbers, Clarke and Beck argue that number perception is abstract and characterized by a second-order character. However, converging evidence from visual illusions and psychophysics suggests that perceived number is not abstract, but rather, is perceptually interdependent with other magnitudes. Moreover, number, as a concept, is second-order, but number, as a percept, is not.

Lucia Schwarz urges a reconsideration of the implications of species egalitarianism, which is an essential element of the position in environmental ethics that Paul Taylor calls “respect for nature.” Species egalitarianism’s claim that every living thing has equal inherent worth appears to lead to counterintuitive conclusions, such as that killing a human being is no worse than killing a dandelion. Species egalitarians have generally responded by explaining that species egalitarianism is compatible with recognizing moral differences between killing different types (...) of living things, and that some killing is morally permissible. Schwarz raises doubts about whether this deflationary defensive strategy is philosophically justified, and suggests that taking seriously the supposedly repugnant implications of species egalitarianism may have a salutary effect on the overall debate. The value of solidarity, which implies mutual concern and support, is often conveyed in our everyday moral and political language. But what is its conceptual relationship with justice? Influential positions in this debate may argue for the opposition between the two concepts: justice is impartial and universal, while solidarity is partial and limited. The present paper aims to shortly explore a range of theories that may exemplify possible answers to this position, from communitarian and realist views, which ultimately confirm the (...) opposition, to political solidarities, which claim an instrumental need for solidarity as a reaction to injustices. Special attention will be paid to Habermas’s compatibilist account of solidarity as the “reverse side” of justice. The paper contends that a compelling answer should preserve the difference between the two spheres and the essential features of solidarity by also sketching a concept of solidarity as a social duty arising from our moral interdependence. The article proposes to sketch an analogy between, on the one hand, linguistic learning and sound dialogue in humanist music therapy and, on the other hand, certain reflections of Maurice Merleau-Ponty: in particular, the centrality of corporeality in the learning, treated in the Phenomenology of Perception and in the courses on the psychology and pedagogy of the child, and the status of inter-corporeality developed through reflections on passivity and the metaphor of flesh. We will trace an analysis of the music (...) therapy of Giulia Cremaschi Trovesi, with reference to the linguistic and musical gesture, through the Merleau-Pontian notion of praktognosia, and of the sound dialogue with reference to institution and intercorporeity. Through an interpretation of music in the terms adopted by Stéphanie Ménasé and in reference to Merleau-Pontian passivity in artistic creation, our objective is to define the musical institution of the vibrating body in music therapy in terms analogous to the institution of the subject according to Merleau-Ponty and, consequently, to find, in the sound dialogue, the intercorporeity which anticipates the individuation of the subject, by describing the communicative authenticity of the musical-therapeutic encounter. Ongoing evaluation of a clinical ethics consultation service (ECS) allows for continuous quality improvement, a process-based, data-driven approach for improving the quality of a service. Evaluations by stakeholders involved in a consultation can provide realtime feedback about what is working well and what might need to be improved. Although numerous authors have previously presented data from research studies on the effectiveness of clinical ethics consultation, few ECSs routinely send evaluations as an ongoing component of their everyday clinical activities. The primary (...) purpose of this article is to equip and encourage others to engage in ongoing evaluation of their own ECS. Toward that end, the following resources are shared: (1) the survey tool used to gather the evaluation data, (2) the procedure used to elicit and collate responses, and (3) how the resulting data are used to support continuous quality improvement and justify the continued financial support of the ECS to hospital administration. Proposed educational outcomes for the health humanities in medical education range from empathy to visual thinking skills to social accountability. This lack of widely agreed-upon high-level curricular goals limits humanities educators’ ability to design purposeful curricula toward clear, common ends and threatens justifications for scarce curricular time. We propose a novel approach to the hoped-for outcomes of health humanities training in medical schools, which has the potential to encompass traditional health humanities knowledge, skills, and behaviors while also being concrete and (...) measurable: humanistic practice. Humanistic practice, adapted from the concept of ethical sensitivity, is an intentional process of applying humanities knowledge and skills to a clinical scenario by 1) noticing that the scenario requires humanities knowledge or skills, 2) informing one’s clinical and interpersonal strategy and behavior with humanities knowledge or skills, 3) reflecting on the effectiveness of the strategy and behavior, and 4) reorienting to develop new approaches for future practice. The construct of humanistic practice may help address some of the foundational problems in health humanities outcomes research since it transcends the traditional diverse content domains in the health humanities, can link patient and provider experiences, and may bridge the divide among the additive, curative, and intrinsic epistemic positions of humanities to medical education. Some recent uses of artificial intelligence for facial recognition, evaluating resumes, and sorting photographs by subject matter have revealed troubling disparities in performance or impact based on the demographic traits of subject populations. These disparities raise pressing questions about how using artificial intelligence can work to promote justice or entrench injustice. Political theorists and philosophers have developed nuanced vocabularies and theoretical frameworks for understanding and adjudicating disputes about what justice requires and what constitutes injustice. The interdisciplinary community committed to understanding (...) and conscientiously using big data could benefit from this work. Thus, in the spirit of encouraging cross-disciplinary dialogue and collaboration, this piece examines contemporary scholarship in political theory and philosophy to illustrate some of the vocabularies and frameworks political theorists and philosophers have developed for thinking about justice and injustice. It then draws on these frameworks to illuminate how the use of artificial intelligence can implicate questions of justice, with a focus on institutional discrimination, structural injustice, and epistemic injustice. Ultimately, the piece argues that the use of artificial intelligence—far from representing a decision to take power out of human hands—represents a novel way of harnessing human power, making questions of justice central to its conscientious undertaking. The general question to be considered in this paper points to the nature of the world described by chemistry: what is macro-chemical ontology like? In particular, we want to identify the ontological categories that underlie chemical discourse and chemical practice. This is not an easy task, because modern Western metaphysics was strongly modeled by theoretical physics. For this reason, we attempt to answer our question by contrasting macro-chemical ontology with the mainstream ontology of physics and of traditional metaphysics. In particular, (...) we introduce the distinction between stuff-ontology, proper of chemistry, and individual-ontology, proper of physics. These two ontologies differ from each other in the basic categories of their own structures. There is a widespread opinion that the realist idea that whether a proposition is true or false typically depends on how things are independently of ourselves is bound to turn truth, in Davidson's words, into “something to which humans can never legitimately aspire”. This opinion accounts for the ongoing popularity of “epistemic” theories of truth, that is, of those theories that explain what it is for a proposition (or statement, or sentence, or what have you) to be true or false (...) in terms of some epistemic notion, such as provability, justifiability, verifiability, rational acceptability, warranted assertibility, and so forth, in some suitably characterized epistemic situation. My aim in this paper is to show that the widespread opinion is erroneous and that the (legitimate) epistemological preoccupation with the accessibility of truth does not warrant the rejection of the realist intuition that truth is, at least for certain types of propositions, radically nonepistemic. Adult humans show exceptional relational ability relative to other species. In this research, we trace the development of this ability in young children. We used a task widely used in comparative research—the relational match-to-sample task, which requires participants to notice and match the identity relation: for example, AA should match BB instead of CD. Despite the simplicity of this relation, children under 4 years of age failed to pass this test (Experiment 1), and their performance did not improve even with (...) initial feedback (Experiment 2). In Experiments 3 and 4, we found that two kinds of symbolic-linguistic experience can facilitate relational reasoning in young children. Our findings suggest that children learn to become adept analogical thinkers, and that language fosters this learning in at least two distinct ways. (shrink)

This is a critical evaluation of the feminist philosophical literature on the work of Emmanuel Levinas. It brought to a close Sandford's research on Levinas, the main outcome of which was her "The Metaphysics of Love : Levinas and Transcendence".

In this paper I outline an account of the structure of perceptual justification that develops Wittgenstein’s thought that the possibility of acquiring any degree of justification for our beliefs depends on placing certain propositions outside the route of empirical inquiry, turning them into the ‘hinges’ of our rational evaluations. The proposal is akin to ‘moderate’ accounts of the structure of perceptual justification, but it conjoins Wittgenstein’s insight with explanationist and contrastivist ideas, and so differs in important respects both from such (...) accounts and from other versions of hinge epistemology. I maintain that the basic presuppositions of the epistemic practice of taking experience at face value are not totally refractory to evidential assessment, arguing that they can receive some degree of second-order empirical justification via inference to the best explanation. And I address the worry that the account may face the problem of ‘easy knowledge’ by helping myself to the contrastivist view that empirical knowledge is best understood as a three-place, contrastive relation between an agent, a proposition (or fact) and a contrast. This leads me to replace the usual closure principles for epistemic justification and knowledge with corresponding contrastive principles, filling in the final details of contrastive hinge epistemology. In a brief overview this article discusses that, in the multifaceted field of cognitive sciences, the problem of consciousness and mind is far from being resolved. The article then argues that this problem is worked out in careful detail in Peirce’s work. Intelligence, hence mind, for Peirce, is distinct from consciousness. Without being dissociable, they are, in fact, distinct but complementary. The COVID-19 pandemic was a global health crisis that required US residents to understand the phenomenon, interpret the cues, and make sense within their environment. Therefore, how the communication of COVID-19 was framed to stakeholders during the early stages of the pandemic became important to guide them through specific actions in their state and subsequently with the sensemaking process. The present study examines which frames were emphasized in the states' press releases on policies and other COVID information to influence stakeholders (...) on what to focus on to help with sensemaking during the crisis. We conducted content analysis on 602 press releases from 50 US states using an unsupervised machine learning approach called Latent Dirichlet Allocation (LDA). The results show that health communication using press releases to help the public make sense of the crisis were framed to include health frames as well as economic frames. Health communication messages are typically framed with health and safety measures; however, this study shows that economic frames were emphasized more than public health frames in the government's health communication for COVID-19, which forced both large and small businesses to engage in specific socially responsible activities that were previously voluntary to support public health safety. Galvano Della Volpe was the dominant philosopher of Italian Marxism for twenty years after the Liberation. His most important book was a work of aesthetic theory—Critique of Taste. Della Volpe, proponent of a robust materialism in all his writings, was concerned to rehabilitate the inherently rational and intellectual nature of art. Opposing both the sociological reductionism of Plekhanov or Lukács, and the formalist irrationalism of Croce or New Criticism, Della Volpe's aim was to demonstrate that conceptual meaning (...) is always inseparable from aesthetic effect. Whether he is discussing Pindar or Góngora, Cleanth Brooks or Roland Barthes, Goethe or Mallarmé, Della Volpe is always challenging, always illuminating. Numenius emphasizes, on several occasions, how the second God (i.e., the demiurge) has a dual nature. In this paper, I investigate the role of the demiurge in Numenius and examine in what sense the second and third God are “one.” On the one hand, Numenius (...) seems to be stressing the unity of the second and third levels of reality, but on the other hand, he also appears to be differentiating them. The present analyses concentrate on fragments 19F, 24F, 29T and 30T (respectively 11, 16, 21, and 22 in des Places’ edition). My purpose is to demonstrate that, according to Numenius, the second and the third God are one because they both can be regarded as demiurgic. Thus, Numenius conceives a kind of “double demiurgy,” which preserves the distinction between the second and the third God, who are distinguished from an ontological point of view, but who, at the same time, share a demiurgic function. The second God is then the paradigm, whereas the third God is immanent in matter as a ruling principle of the cosmos, which is similar to the World Soul, as he operates on matter in order to make it rationally ordered.
